The ledger is laid out as ‘Receipts’ and ‘Payments’ with monthly balanced totals at the bottom. ‘Receipts’ entries are listed under the following headings: ‘Date’; ‘Total’; ‘Subscriptions and Donations’; ‘Other Voluntary Gifts’; ‘Private Patients’; ‘Out Patients’; ‘Approved Societies’; ‘Other Sources’; ‘Public Authorities’; ‘Nurses and Probationers’ Fees’; ‘Government Grants etc.’; ‘Interest and Dividends’; ‘Rents’; ‘Farm Receipts’; ‘X-Ray, Massage and Electrical Receipts’; ‘Bequests’; ‘City Collector’; and ‘Sundries’. The headings ‘Bank Lodgements’ appears from January 1934 onwards, and ‘In-Patients’ appears from 1937 onwards.

There was different coloured ink used for each heading from 1937 in order to differentiate between entries. Headings also change order throughout the course of the ledger with some crossed out and new ones added in such as ‘National Health Insurance’ which appears from 1938 onwards. On the final two pages of the book are one year totals for both income and expenditure listing October, November, December, 3 month, 9 month and complete totals for both sides under headings such as ‘Receipts for Services Rendered’, ‘Invested Property’, ‘Extraordinary Expenditure’ , ‘Surgery and Dispensary’ and ‘Administration’.
Under ‘Receipts’ all securities, investments and bequests are named, whilst under ‘Payments’ all companies and businesses used are named, with the reason for payment noted beside.
