In some cases both parts are still present, some time with one or both filled in, in some cases the completed countefoil has been removed. Records contain the following introductory information; name, address, occupation, age, sex, marital status, hospital, date of admission, date of death, under care of, date of Post mortem and post mortem conducted by, clinical history, diagnosis and manner of death. The record of the post mortem results is divided under the following heading; external, head and spine, thorax, abdomen, limbs, special notes, nature of diseases, microscopic appearances and cause of death. |
