There is an alphabetical index of patient names at the front of the register with corresponding entry page number. Each patient’s case is listed under the following headings: Bed; Name; Residence; Date of Admission; Married; Age; No. [Number] of Children; No. [Number] of Abortions; Time Since Last Pregnancy; By Whom Sent In; History; Menstruation – Symptoms, Regularity, Amount, Pain; Leucorrhoea [white or yellow vaginal discharge] – Character, Amount; Pain – Locality, Degree; Other Symptoms; Physical Signs; Diagnosis; Indications for Operation; Operation; Result; [When] Discharged.
Some patients’ case notes also include a printed and handwritten ‘clinical chart of temperature’ pasted onto the opposite page of the notes which shows the patients temperature in Fahrenheit, pulse and respiration over a certain number of days, and also lists their name; age; and disease/procedure. Certain cases also have manuscript follow-up notes written on the opposite page. No named attending surgeon or physician is shown within the case notes, although the Medical Board Minutes suggest it was gynaecologist Dr Robert P Farnan. Also contains rough-hand clinical sketches and diagrams within some case notes.
Page 199 has a single blue notepaper page pasted into the spine along with the clinical temperature chart consisting of a short medical report by Dr [Robert] Farnan on the patient’s condition following an operation. |
